Internal Anonymous Feedback and Recognition Form

This form is intended for internal use by members of the Rochester Volunteer Fire Company. It provides a way to share constructive feedback — positive or negative — and to recognize a fellow member for actions that reflect well on our department.

Submissions are reviewed by department leadership. Because this form may be submitted anonymously, you should not expect a direct response unless you choose to provide contact information. Feedback trends and concerns will be evaluated and addressed when appropriate.

Expectations for Use: This form is for professional, constructive communication. Be specific, factual, and respectful. It is not to be used for personal attacks, retaliation, gossip, or inflammatory comments. If you are recognizing a member, please include enough detail to clearly describe what they did and why it mattered.

This form does not replace the chain of command. Members are encouraged to bring concerns to their officers whenever possible. If you are not comfortable doing so, this form exists as an additional avenue for communication.

If the matter involves an immediate safety concern or urgent issue, contact an officer directly.
